Yellow Dot Program
A Yellow Dot sticker in the lower left corner of the rear window of your vehicle will alert all first responders to the vital information in your glove box compartment. This will help to ensure that you receive the medical attention you need during that first "golden hour" after a crash or other emergency involving your vehicle.

Three items are needed to participate in the Yellow Dot Program:

Completed Personal Medical Records Form

A photo to attach to the Medical Records Form

First, attach a photograph of the participant in the box on the Medical Records Form.
Next, complete the form which contains the participant’s medical history, allergies, medications, dosage and names of doctors caring for the participant, along with the name, address and phone number of a person to contact in case of an emergency.

Next, attach the Yellow Dot in the lower left corner of the rear window of the participant’s automobile on the driver’s side to alert first responders that vital medical information can be found in the glove box compartment of the vehicle.
Finally, place the completed Medical Records Forms, with the photograph attached, in the glove box compartment.
